Appointments

Patients are seen by appointments that can be scheduled by calling our office. Office hours are 8:30 - 5:30 Monday through Friday. Special arrangements are made for patients with unique scheduling needs!
Emergencies and urgent requests are always accommodated.
Walkin appointments accepted acording to availabllity.

Emergencies

Emergencies can be managed by calling our office at any time. If an emergency occurs when the office is closed, Dr. Levine will be paged and return your call promptly to provide the appropriate care.

Your First Visit

Your first visit will include a complete medical history review and thorough physical examination of your lower extremities. X-rays and laboratory may be necessary to properly diagnose and treat your condition. Every effort will be made to provide you relief of any discomfort and to hasten your recovery.
You will be asked to complete a medical questionnaire and a patient information form. Please bring a list of your current medications and your physicians name, address and telephone number. It is our policy to inform your physician of your ailment and treatment so that we can coordinate your medical care. Upon request, these forms can be mailed to you prior to your initial visit. Fees

The fee for your care may vary according to the complexity of your condition. All fees and servicies will be fully explained to you when you are in the office. If an advanced procedure or surgery is considered, we will contact your insurance company to verify and authorize your care.

Insurance

Our office is a provider of most insurance compaines. We are also participating physicians for most Preferred Provider Organizations (PPOs), as well as Medicare, Point of Service (POS), and many HMOs.
Payment of the charges are the responsibility of the patient. Our office staff includes a full time insurance billing department and is equipped and prepared to directly bill your insurance company for you or to provide you with the necessary paperwork.
